What You'll Do:
Provide compassionate, high-quality care to patients in nursing home settings
Conduct assessments, diagnose conditions, and develop personalized care plans
Collaborate with physicians, nurses, and healthcare professionals
Prescribe medications and educate patients and families on wellness and preventive care
Maintain accurate, up-to-date patient records
Follow all regulatory and organizational guidelines

What We're Looking For:
Master's Degree in Nursing (MSN) from an accredited program
Active New York State Nurse Practitioner license and Board Certification
Experience with eClinicalWorks EMR preferred
Strong communication, problem-solving, and teamwork skills
New graduates are welcome and encouraged to apply
