POSITION SUMMARY:

This position supports the work of the Y, a leading nonprofit, charitable organization committed to strengthening community through youth development, healthy living and social responsibility. The Case Manager is responsible for all Housing Case Managing functions that deals directly with the stability and progress to a healthier community life of all the residents, including providing programs and resources that enable residents to grow, develop and become productive members of society.

Job Summary: The Case Manager is responsible for coordinating and overseeing the delivery of services and support to clients in various settings, including healthcare, social services, or community organizations. The primary goal of a Case Manager is to ensure that clients receive appropriate services and care to meet their individual needs, while also helping them achieve their goals and improve their quality of life.

Key Responsibilities:

  • Client Assessment: Conduct thorough assessments of clients' needs, strengths, and challenges to develop personalized care plans.
  • Case Planning: Develop, implement, and monitor individualized care or service plans in collaboration with clients and other service providers.
  • Resource Coordination: Link clients to appropriate services, including healthcare, social services, housing, employment support, and mental health services.
  • Client Advocacy: Advocate for clients to ensure they receive the appropriate services, treatments, or accommodations required.
  • Monitoring & Follow-Up: Continuously monitor clients' progress, adjusting care plans as needed, and provide ongoing support and follow-up.
  • Documentation: Maintain detailed and accurate records of all client interactions, progress, and service delivery in compliance with organizational policies and regulatory standards.
  • Collaboration: Work closely with interdisciplinary teams, including social workers, healthcare providers, and community organizations, to ensure comprehensive client support.
  • Crisis Intervention: Provide support during crises and help clients navigate difficult situations, including safety planning and emergency response.
  • Education & Support: Educate clients and their families about available resources, rights, and services, helping them make informed decisions.
  • Compliance: Ensure adherence to legal, ethical, and organizational standards in all case management activities.

Qualifications:

  • Education: Bachelor's degree in Social Work, Psychology, Nursing, or related field. (Master's degree may be preferred for certain positions)
  • Experience: Minimum of 2 years of experience in case management, social services, or a related field. Experience in a healthcare, mental health, or social services environment is highly preferred.
  • Skills:
    • Strong interpersonal and communication skills.
    • Excellent problem-solving and critical thinking abilities.
    • Ability to work independently and as part of a multidisciplinary team.
    • Knowledge of local community resources and services.
    • Organizational skills and attention to detail.
  • Certifications (if applicable): Case Management Certification (CCM) or other relevant certifications may be required or preferred.

Work Environment:

  • Full-time position with occasional evening or weekend hours depending on client needs.
  • Travel within the community may be required.

Work in a collaborative team environment with support from supervisors and colleagues.

Obtain Criminal background check, Child Abuse Clearance, and FBI fingerprint clearance.
